Benefits Enrollment Extended

The enrollment process for personal benefit elections is now
being conducted exclusively via Touch-Tone telephone.
Personalized Telephone Election Worksheets were sent to home
addresses in late October along with the Personal Benefit
Elections Workbook.
To enroll, call 516-2367. From outside Baltimore, call
1-800-548-6968 or 1-800-JHU-N-YOU. The Personal Benefit Elections
Line is available through Nov. 30, from 7:30 a.m. to 9 p.m.,
seven days a week. Availability of the Personal Benefit Elections
Line will be extended from Dec. 1 through Dec. 30, Monday to
Thursday from 7:30 a.m. to 9 p.m., and Friday from 7:30 a.m. to 7
p.m.
The line will be available in a limited way or not available
on these dates:

     Thursday, Dec. 1 7:30 a.m. to noon
     Friday, Dec. 23 7:30 a.m. to noon
     Monday, Dec. 26 not available
     Friday, Dec. 30 7:30 a.m. to noon

Please note that elections or enrollment made after Nov. 30
will be effective Jan. 1, 1995. Some individuals may experience a
delay in receiving membership cards or being on record with
insurance companies for any newly elected plans. This may delay
any claims being processed for services after Jan. 1.
Enrollees will be required to identify themselves by Social
Security number and personal identification number. The PIN
number is printed on each Telephone Election Worksheet. Do not
return the Telephone Election Worksheet.
A confirmation statement will be mailed after the telephone
election is made. Anyone who chooses not to use the Personal
Benefit Elections Line to make a 1995 election will have default
coverage; 1994 benefit elections will be maintained at 1995
rates.
